Update the Entity Item import process for new UPT fields

Update EntityItemImportValidator, EntityItemImportMapProvider, and EntityItemImportProvider to handle new field and file formats.

EntityItemImportProvider needs to set the new Use UPT Import configuration for a store, process either csv or xml versions of the file, and use AutoCalc's value on the item to decide if the UPT should be updated.

This will probably require a new stored procedure to update UPT to the imported value.
