Item Activity Page is showing incorrect user

Item Activity Page is showing incorrect user, for restaurant # 02621:

Please investigate why a user that is not an employee at this location shows on the Item activity report for a waste entry.
The user Michael Bendixen can be seen as the user who entered waste from the POS. 
I have confirmed that Michael Bendixen is not an employee at 02621 nor are they on the terminated employee list.

Select * from tbUSer with ( nolock) where UserID = 1055817

I see that some stores are using the operator 'TakerTwo, Order' with ID = 1055817,  I see  the same issue for the other stores as well, the following is a screenshot from store 03008
