As a user, I would like to see "Select report group", "Select report", "Report details" controls on the left side of the screen.

Confirm "Select report group" is populated.
Confirm "Select report" is populated based on selection from "Select report group"
Confirm "Report Details" are populated based on report selected.
- The "Report Details" section is collapsed by default but the user can open it if they choose.
- Confirm Notes section is present if Notes has a value
- Confirm the Report Format exists
- Confirm Active Report exists and the checkbox is checked\unchecked based on report selection
- Confirm Queued report exists and the checkbox is checked\unchecked based on report selection
- Confirm Report ID exists
