As an admin, I would like there to be audit logging for the Sales Exclusions UI.

- The username is recorded when a sales exclusion is created.
- The username is recorded when a Sales Exclusion is edited.
- Deleted Sales Exclusions are recorded including the username of the deleting user.
- Recording this information does not mean that a user can not create a new sales exclusion with exactly the same details as the deleted record.
