Update Count History for Monthly Counts to display period information

      Description

      Story:
      As an Operator, I would like the ability to view the period a monthly count was performed when viewing the Count History header and detail pages.

      Details:
      Add the period a monthly count was performed on the panel heading of the monthly count detail screen and append the period to the monthly count type for the count history header page.

      Acceptance Criteria:

      1. Confirm the period a monthly count was performed is displayed on the count history header and detail screens.
      2. Confirm the period reflects the correct period the count was entered.
      3. Confirm the user cannot change the period.
      4. New - 10/2 - Display Month in grid of Count History Page, for example Monthly - September 2018
