Update Review Count Screen to display count period (month name and year)

As an Operator in InFORM who performs monthly counts, I would like to see period information for the count I am performing.

Details:

• When creating a monthly count, go to Actions > Review Count
• Display the period (Month YYYY) in the header
• 

Acceptance Criteria:

1. Confirm when reviewing a Monthly count, the period (month and year) is displayed on the header bar.
2. Confirm the month and year captured are the month and year that I am currently counting.
