As a product owner I want the work performed in CFAMX-1299 to be rolled back so that the Menu Item Cost import is not processing the Fair Market Value

Acceptance Criteria
- Confirm that the Menu Item Cost Import is updated to NO LONGER include the Optional field for the Fair Market Value
- Confirm that the validations are updated to no longer look for the Fair Market value
- Confirm that if the Fair Market Value is included in the header record, it is not processed
- Confirm that the Menu Item Cost Import passes all regression tests
